Who do you put for the debtor if the debt has been sold on?

I have a number of credit cards where the debt has been bought by a debt recovery agency.
If I'm going bankrupt, what name of debtor should I put on the form? Does your credit file have both names on?
Any advice would be grateful.

In the first section you put who the account was opened with. Then you get to another section where you have to list them all again, in this section you put who they are currently with. If you don't know just put the original bank.
