What tax code is on your P60 - chances are the medical benefit in your tax code was less than the actual amount per P11D and you have underpaid tax on your PAYE as the tax alone on your self empployed profit would only be £24 ish.
In fact looks like your medical benefit wassn't included in your tax code at all and a further £249 @ 22% is due on this0 -

The 'normal' code for 2007-08 was 522L so if yours was 519L then only a restriction of £30 appears to have been made for your medical benefit (probably based on the previous years figures) so part of the £75 underpayment relates to the fact your actual medical benefit was greater than that in your code0

To avoid this happening next year, check your current year tax code shows the correct deduction for medical benefit.0
