Cause of Bankruptcy

Me & OH are going BR on Wednesday.
Do we have the same "cause of Bankrupctcy" statement?
This is a first draft:
"My husband and I have always made credit card repayments on time but began to find difficult when my maternity leave commenced in 2006. Statutory Maternity Pay equated to only half of my usual income.
I had no option but to return to work on a part-time basis but childcare costs put us under considerable financial strain. Further to this I underwent surgery in November 2007 and the time I had to take off from work resulted in a reduced wage.
In addition, we found that increasing energy, food and petrol prices further reduced any disposable income we once had and we soon became aware that we were living on credit.
In an effort to reduce crippling interest charges, we took out a large consolidation loan but further increases in nursery fees, school meals & household bills, and a reduction in tax credits due to an overpayment that we were not made aware of, meant we were still using the credit cards for necessities. It began to get harder to meet minimum repayments.
We then sought advice and believe that bankruptcy is our only option to stop mounting credit charges and once again take control of our finances."
Any advice please?
