Terms And conditionsAccidental Death Insurance Terms and conditions
This policy is designed to meet the needs of associates of BHSF aged over 17 and under 70 who want to ensure that their family or estate will receive a lump sum benefit if they should die as a result of an accident.
Underwritten by Avon Insurance plc. This certifies that Avon Insurance plc (the Company) will provide insurance in the
terms of the Certificate to be supplied from the Effective Date.
Eligibility
You are eligible for this £5,000 free cover if you are aged 17 or above and under 70 years and residing in the United Kingdom at the start of the policy.
Benefit
The Company will pay to the estate of the Insured Person the Benefit upon death of that Insured Person, provided that death has resulted from, and within 365 days of, an accidental bodily injury occurring during the period of insurance of the Certificate. The amount of free Benefit will be £5,000. The Benefit is non-assignable. This Certificate has no surrender or maturity value.
Accidental Bodily Injury
Accidental bodily injury means bodily injury resulting solely and directly from accidental external violent and visible means and does not include illness or disease or any naturally occurring condition or degenerative process.
Insured Person
The Insured Person means the Insured named, as stated in the confirmation letter to be supplied.
Exclusions
The Company will not pay the Benefit for death resulting directly or indirectly from:
(1) suicide, attempted suicide or intentional self-injury;
(2) an accident occurring while the Insured Person is under the influence of alcohol or of any drug not prescribed by a
registered medical practitioner;
(3) flying or any form of airborne aerial activity, except while traveling on a recognised airline;
(4) war, invasion, or act of foreign enemy, hostilities (whether war is declared or not), civil war, rebellion, revolution,
insurrection or military or usurped power.
Cancellation
The insurance provided under the Certificate to be supplied shall be cancelled automatically upon the date of the occurrence of the earliest of the following events:
(1) the end of 1 consecutive year of cover.
(2) on the date the insured attains age 80.
You have a right to cancel your policy for a period of 14 days from the date you receive confirmation that your policy is in
force.
If you wish to cancel your policy you should write to:
PA Plans, Avon Insurance plc, Arden Street, Stratford-upon-Avon, Warwickshire CV37 6WA.
Notice and proof of claim
Notification of claims should be made in writing, within 365 days of the date of the death, to: PA Plans, Avon Insurance plc, Arden Street, Stratford-upon-Avon, Warwickshire, CV37 6WA. Telephone: 01789 200 855.*
The Company, upon receipt of such notice, will provide forms for filing proof of any claim. Written proof of accidental bodily injury must be provided to the Company.

In the event that you remain dissatisfied, the Financial Ombudsman Service may be prepared to review your complaint. No charge is made for this service. The Financial Ombudsman Service, South Quay Plaza, 183 Marsh Wall, London E14 9SR. Telephone 0845 080 1800. The Ombudsman’s decision is binding upon Avon Insurance plc, but you are free to reject it, and doing so will not affect your legal rights.
IMPORTANT INFORMATION
The Certificate and the confirmation letter constitutes the contract between the Insured and the Company. Insurance is only effective if the Insured is eligible for insurance and becomes and remains insured in accordance with the provisions, terms and conditions of the Certificate to be supplied.
As part of our commitment to customer service we will contact you about your Policy and our service before the cover ends.
The law applicable to the contract: The contract and the relationship between Avon Insurance plc and you shall be
governed by, and interpreted in accordance with, English Law. The contract shall be subject to the non-exclusive jurisdiction of the English Courts.
Benefits are paid free of personal income and capital gains tax, based on our understanding of current legislation. The benefit may be subject to Inheritance tax.
Data Protection Notice
You consent to us using your personal data, including any sensitive personal data ("Data") that you have provided to us, or to third parties or intermediaries on our behalf, to set up, provide and administer your policy, including for underwriting, claims handling, fraud prevention and research purposes. You agree that this could involve passing your Data to regulatory or other organisations and public bodies including the police, or to other countries inside or outside of Europe. If we need to do this, we will, if appropriate, arrange a contract to make sure your Data is adequately protected in accordance with the Data Protection Act 1998 and EU Directives.
You consent to us using the Data to send you information about other Avon products or services, or products or services of other organisations that we think may be of interest to you. You also consent to us passing the data to BHSF Services Ltd to enable them to send you information about their products and services. Financial Services Compensation Scheme
Avon Insurance is covered by the Financial Services Compensation Scheme (FSCS). You may be entitled to compensation from the scheme if we cannot meet our obligations. This depends on the type of policy and the circumstances of the claim. Compensation payments are limited so the protection offered by the scheme is not total. The scheme covers 100% of the first £2,000 and 90% of the remainder of the claim, without any upper limit. You can find out more by visiting the FSCS website www.fscs.org.uk or by telephoning 0207 892 7300.
Avon Insurance plc is authorised and regulated by the Financial Services Authority for general insurance.

The contract and other documentation are drawn up in the English language. We will communicate with you in writing in English throughout the duration of the policy.
BHSF which is authorised and regulated by the Financial Services Authority chooses to introduce to Avon Insurance plc for the purposes of arranging accidental death cover. Avon Insurance plc only offers its own products for accidental death insurance.
