Change of address template anyone ?

Hi all, I just read on an old post that there is a template/letter that can be used when notifying DCA's of a change of address. Does anyone have it please ?
Also any suggestions/comments before I do this, like will a new address be accepted easily ? Think I also read somewhere in this forum that there is an official line/law that says DCA's must accept your new details ?? Why wouldnt they ? Bit confused !
Desperate to get this done next week so ANY feedback re change of contact details greatly appreciated.

Just draw up a wee letter & copy it & send it off.
