Equifax specific CRF clean up question

I have been reading through the excellent CRF clean up document in the stickies, very very helpful.
Call credit is as promised totally wrong, but I am currently working on the Equifax side of things. In my records it states the following, is this what it should say as it is slightly different to that shown in the sticky:
Recorded at:XXXXXX
Account holder name:Mr X
Date of birth:2xth Max 19xx
Company type:Financial Services
Account type:Credit Card
Last updated:7th November 2007
Current balance
Account closed on 31/10/2007 with an outstanding balance of £2,814.00
Credit limit:£3,000.00
Opening balance:£0.00A
ccount start date:12th June 2006
Regular payment amount:Not Applicable
Number of repayments:Not Applicable
