Employment Law advice needed please

I really need some advice from anyone who may have experienced this.

I have been on work related stress for 5 months now and my boss hasn't done anything to resolve my grievances. I have now been offered another job which I really want but the new employer has asked me to complete a medical questionnaire to assess whether I would be suitable for the new position. If I answer the questions honestly and say that I've been off work with Work Related Stress can they withdraw the offer? The Work Related Stress has been due to bullying, harrassment, discrimination and breach of privacy.

Any advice please?

Well the new job will want references from your old employer so they will find out anyway and sack you for lying.

If the offer to you is subject to satisfactory medical (and references?) then the offer is conditional and can be withdrawn if they are not satisfied.
