help needed

hi, i'm currently trying to fill in a interview pack from payplan. i'm on the expenditure part and lost. any chance of someone posting a rough copy of all the stuff i'm meant to include.

also i live with my gf but she doesn't want any part of this. I pay most the bills, rent, etc and she pays shopping and for the sky. do i put all of this on the expenditure. lastly, seems she doesn't want any part of it, do i put that i all the stuff or what ? she only earns a little each week so really at a lost of what to put.. please help me out.

    Include everything that you pay out each month - so if you pay half the rent, put that down. Likewise with food, household bills etc etc. Do you have a joint account or seperate?
