BR forms

Hi , i have been filling in forms for going BR but i have a couple of qs.
Firstly when you fill in who you owe, do you put the original people you owe or the company who now have the debt?
Secondly is it important to fill in the address etc for the debts?
and lastly when i have tried to put in my outgoings and the section that says any other , i was hoping to put in things like haircuts/activities but it wont allow me to put in amounts/ what should i do

    No idea about the second question. Are you filling it in online?
    As to the question about creditors, is there space to put the most recent "owner" of the debt in brackets after the original? Re addresses, head offices (as printed on statements) should do. Chances are that your OR will have the relevant departments of most major lenders on speed dial!
