I have a number of application forms to complete. All these forms ask for my "Employment Details Previous or last Employer". Until recently i was self-employed so I was the previous employer. Do i put in my previous employer from four years ago or myself? Thanks for any help or advice.

Is this in the section where you provide referee details? If so, go back to your previous employer.
If it is in the employment history section, they are looking for what work you have been doing. In this case write "self-employed" or use your trading name if you used something other than exactly as your name appears on the form.0

Just put self employed in the company name, the address you traded at, the date you started and finished and in the job description give a rough overview of what you did.0
