Anyone got a link or knows where to get the right forms from on the oft website have trailed around it for ages and only found one form but it states that it is for advice agencies and third parties to use i want one for me to use anyone any ideas please help

Not sure what forms you are referring to magics.
For consumer stuff, they have this site - don't know if this is what you need.
