i need a template letter for getting the remainder of my front loaded ppi back after

Hi all can anyone point me in the right direction of a template letter(i am sure there must be one!)to ask for the remainder of my ppi back from a front loaded loan that we cancelled ppi on?? cccs said we need to write to them but didn't actually give us a ltter to use or anything!
and i am a bit stuck!
