    I've never used a form, hun. Just sent the vouchers with a letter with the title of the experience, the code, and the value and asked for them to send any balance back to moi. :D

    sure it would if you have all the codes, cant see there being a prob.. perhaps just mention you didnt receive an order form with voucher?
