It was me! I'm not sure what should go in there because the boxes were blank but it seemed to make sense and both claims went through OK - haven't heard anything back yet though but that seems to be the norm. Perhaps the next winner who has labels could let us know what's supposed to be in the boxes.

Oh and I just remembered there was another tick box under the comments asking for permission to publish them which I didn't tick because I didn't make any comments.
