            Dont know where the form is now, probably filed in HR somewhere. I was never told if the form was approved or refused, as never recieved a reply, but I know the form was taken from the box as checked it every week for a reply slip.

            . . . we fill one in and put it in a holiday forms box, stating which department we work on, the date we want the holiday, and the reason for the holiday.
 If it is a form that relates only to paid holiday rather than other absence requests, they seem to be asking more than they need to know. Do they use it unfairly to decide whether to allow the holiday or not?0

            I think you really need to chase this up....if what you say is correct, there's been a serious failing on your immediate supervisor/management and/or your HR department.
 I would ask for the form, if it cant't be produced.....someone is telling porkies....in the absence of a form...it could be a sticky one....so hope its there...I'm now a retired teacher... hooray ...:j

            I never chased it up was completely knew to employment and didnt really understand how the holiday forms worked, so I thought that it had been dealt with and I just hadnt been given a reply slip, but I then knew this wasnt the case when mentioned it to my supervisor.
 The Holiday form is for any day that we want off, I know that it doesnt have to be specifically a holiday, as I did the same last weekend (half of which was put on this months payslip) and got paid for it.
 I will have to contact ACAS this week and see what they have to say about the situationNever knock on death's door, ring the doorbell and run away ..... he hates that :mad:0
