need a income and expenditure worksheet

Hi can anyone point me to a income and expenditure worksheet that i can fill in and print off to send to all my crediters.There's one on the national debtline but this only lets you fill in upto 9 when i need about 13 lol.Anyway thanks in advance toffee

The CCCS "Self-help" pack includes standard Income & Expenditure forms, which are clearly set out and which the creditors will be used to seeing.

have you tried going to your local CAB (Citizens Advice Bureau). They do all that for you.
