CCA Legal?

Hi
Please can someone tell me if this CCA is legal:

It took me 3 months & 3 official letters to get it during which my request was in default but they kept charging me interest & late payment fees. Is there any ways to get these back & should I stop paying as the CCA has no signature only a tick box that doesnt even state my name next to it.
My name & address were on the 1st page of the document & thats all.

Tick boxes are now legal but I am not sure of the exact date when this became a legitimate means of signing a CCA.
The other main elements are included; limit or means of decidig this, repayment terms and APR.If you've have not made a mistake, you've made nothing0
