My Debts Are In My Name And My Partners

I received my NDL pack today, when I spoke to them on the phone I told them that the debts I have got are in both of our (differant) names, (mine and my partners)but they have just filled out the form in my name with OUR incomings and outgoinings and they have got some of it wrong. I am sorry to be thick, but what do I do, I am lost and scarred.
I am 70% sure that I am going to go through with it.
Any help and advice would be gratefully received.
