Here at Community Care we aim to continually improve our magazine for everyone who reads it.
To help us with this we would like to invite you to complete a short, confidential survey telling us what you think of our magazine and a little more about yourself. The results will be used to ensure we are providing you with relevant and interesting content.
The survey should only take about 5 minutes to complete. As a thank you for taking part, all completed questionnaires will be entered into a prize draw to win £1000.
