We, the Tourism Authority of Thailand and the InsightAsia Market Research Co.,Ltd, are currently conducting a survey on the on-line distribution channel of Tourist information. Please kindly spare 10-15 minutes of your time to fill out this on-line survey. For this particular study, we would like to focus on residents of the following countries, namely Australia, USA , United Kingdom, South Korea, and Japan)
Upon completion of survey, we will randomly draw a 5 lucky winners to get the prizes for 2 pax each, which include FREE airline tickets from Bangkok to Phuket (round trip) and 3 days 2 nights stay at a luxury beach resort.
Winners' detail including
1) The prizes are open for entry to the residents of the following countries (Australia,USA,UK, Japan, and South Korea)
2) All entrants and winners must be at least 18 years of age
3) The prizes are not open to marketing researcher, travel agency or any travel related business including tourism authority/organization/association of any countries
4) Closing date for all entries are 15 September 2008
5) Each prize is for 2 persons in twin or double room and subject to availability, advance reservation is required at least 15 days . Prize validity from May till October 2009.
6) Acceptance of the prize constitutes consent to use the name and likeness an those of his/her companion for editorial, advertising and publicity purposes. Tourism Authority of Thailand reserves the right to publish names of winners in relation to editorial, advertising and publicity purposes.
7) All winners will be notified via e-mail. If the winner does not respond to the notification within 29 September 2008 , another winner will be chosen. Failure to respond 29 September 2008 shall mean that the winner forfeits the prize.
8) Tourism Authority of Thailand reserves the right to alter, amend or close the survey if necessary. The prizes are not transferable. No substitutions or cash alternative for the prize will be allowed.
9) Only one entry per e-mail address and per individual will be considered for the prizes entry
10) Neither Tourism Authority of Thailand , nor any agencies involved in this online survey will be liable for loss/damage whatsoever caused by matters arising from this survey.
11) You acknowledge and agree that information submitted by you may be made available by TOURISM AUTHORITY OF THAILAND for viewing, rating, reviewing, and commenting by the public.
12) You agree to hold TOURISM AUTHORITY OF THAILAND and InsightAsia Market Research Co.,Ltd. and its officers, directors, employees, agents, successors, and assigns harmless from and against—and hereby waive any right to pursue—any claims of any nature arising in connection with TOURISM AUTHORITY OF THAILAND's use of the information submitted and used in any manner in TOURISM AUTHORITY OF THAILAND's sole and absolute discretion.
13) Selection of the winner will be randomly picked entirely in the sole and absolute discretion of TOURISM AUTHORITY OF THAILAND , the odds of winning depend upon the number of entries received and cannot be determined in advance. Decisions by TOURISM AUTHORITY OF THAILAND are final.
14) Winners may not request substitutions of prize winnings. All winners are solely responsible for any and all taxes and/or fees as well as all additional costs that may be incurred.
15) Neither TOURISM AUTHORITY OF THAILAND, any agencies nor its officers, directors, employees, agents, successors, or assigns shall be liable for any warranty, costs, damage, injury, or any other claims incurred as a result of the usage of a prize by any winner. TOURISM AUTHORITY OF THAILAND is not liable for any loss arising out of or in connection with any activities promoted by TOURISM AUTHORITY OF THAILAND.
16) TOURISM AUTHORITY OF THAILAND reserves the right to require the winner to execute an affidavit of eligibility as a sworn document and provide other proof of eligibility as a condition of receipt of the prize or award.
17) If the specified prize becomes unavailable for any reason, TOURISM AUTHORITY OF THAILAND in its sole and absolute discretion may substitute a prize of like or equal value.
18) Entrant is null and void in any jurisdiction where prohibited by law
19) TOURISM AUTHORITY OF THAILAND reserves the right in its sole and absolute discretion to terminate any entry at any time without prior notice.
