Firstly I am no expect my replies include what I have learnt about the system from our dealings with them and there webpages. So don't take what I say as gospel.
You Just have to keep pushing them. I can't get my head around why some people have major problems. We must be lucky at have a very good team.
We on the old system and where told 13 weeks for a re-assessment and that with in the last month and got it in about 4 weeks. We where rather gob smacked to say the least. But at least it meant we where not running up back pay for months.
I have feeling that they can't string an assessment for over year. As the intial assessment is only suppose to take 26 weeks 6 months. And we had the problem of them taking longer and racking up more back pay and my husband fought it got the extra weeks removed as it was not are fault.
I think you really need to talk to a supervisor or manager and put in a offical compliant.
Have you read this may help.
Also when ever you speak to someone at the CSA always take there name and note the time and date down. As they can forget to make notes on the computer system them did the other week for my husband told them he would be making a payment and the rang him up to ask why this money had come in to his account with them.
Not sure what else to say apart from keep pushing and if you can try and ring during the day as your casework is more likely to be there.

2) having the children to stay less nights
3) you as wife/partner refusing to give wage info -personally I find it highly offensive as on the new system the wifes/partner income is not taken in to account and neither is the PWC.
Having all this knowledge does not help when you are banging your head against a brick wall trying to get things sorted out.
As I said before if you want to PM then do so by all means. It is very hard to cope with this alone. And if at least you know someone else who has or is dealing with it, it does help.
Also I am sure that you are aware that if you have children of you own you are could be entitled to tax credits and they will taken in account with the CSA when the time comes.
