I noticed camelbak water carriers recently at my local outdoor shop which have a pipe that can be clipped to clothing. There are lots of different ones that can be put in bags and some have their own rucksacks. Used for active sports mostly but may solve a problem.
The pipes have a valve that won't let the liquid leak out.
Other manufacturers have similar solutions maybe cheaper.
Looks like their bottle have a non-leak valve too.

Woolworths had/have some large sports bottles with a strap in their picnic and camping section - they're half price right now at £2.49
It's this range, although the insulated sleeve is different in Woolies - the strap is fixed up one side so you can slip your hand between it and the bottle to aid the grasp. I got a few for my elder boys at school and hubby as they all can have grasp problems at times.
