CCA Unecforceable questions

I've done my research through these forums and am i right in thinking that if you send a request to the creditor asking for your CCA and they dont get back to you within a certain amount of time the account goes into dispute or on hold?
Then what steps make it uneforceable? Also what happens if they do provide the agreement? and how does one know if its the actul agreement or just a "fob off" letter?
and lastly has anyone ever suceeded in this?
