That's because small businesses tend to take a conservative attitude. They think they're God's gift to the community, and consider themselves charitable for having given you the job in the first place.
Big businesses started out as small businesses. There are a lot of small businesses that like to think they provide a useful service to the community. However, only a few grow big - and they're the ones who really do provide a useful service, such as food, fuel, and non-fashionable clothing. The small businesses who don't provide a useful service to the community stay small.
But that's not the only reason why it's bad to work for small businesses. If you work for a small business, then your boss, and the person who interviewed you for the job in the first place, will probably be the owner of the business. A small business naturally cannot achieve the economies of scale that bigger businesses can get - so the only way he can continue to trade is to appeal to the conservative attitudes of the middle-to-upper classes, who have more money than sense, who believe that shopping at Harrods rather than Matalan is a point of principle.
However, the human resources function of biigger companies tends to be run by employees. And if you get the job, your line manager will also be an employee. Everyone you deal with in a bigger business will be an employee - a worker - and therefore also on your side. No-one has a bad word to say about Ford in the 1920's. I genuinely believe that big business is good for community cohesion, and that small businesses cause social exclusion. The reason why McJobs are so bad is because McDonalds usually trades as a franchise. Individual McDonalds branches are usually small businesses disguised by big brand names, but who take the same conservative attitudes found in any small business.
Sorry for the rant - but I hope I've provided food for thought - and I hope he gets the sick note, we're all rooting for you. In the very unlikely event that the doctor refuses to give a sick note, I'd suggest trying another doctor at the same surgery.0
