We had a book that all refunded people had to put their name and address in. I always presumed so that staff could not make up refunds and scam the company.

I paid for an item (using cash) at Jollyes pet store, as the assistant handed me my change I noticed they had overcharged me by something like £1. She called the manager over who demanded that I fill in a form with all my personal details for my 'refund'. I refused out of principle saying that it wasn't a refund, I hadn't even left the shop let alone the till point and that all she was doing was giving me my pound back that they had overcharged me!

I won although she did insist on a signature, gave them my best squiggle :D
Making you do that for a £1 overcharge is a bit daft though. They should have voided the transaction and done it again properly. Maybe ring head office or email them if a bit miffed and who knows they might give you something.

From what i remember it was to stop staff putting through non-existant refunds and taking money from the till. never got my head round how it worked it out though, you could easily override it when I worked there.
