Draft leter for finding out old debts

Please could somebody post a draft letter that I can copy when writing to creditors to find out about the debts I still have with them.

I know who I have the debts with originally (but don't know if they may have been passed on) but do not know any other info such as references, amounts and who to contact to arrange repaying them.

So I need a letter to send to the companies asking for this info, can anyone please post one or link to one on the internet.

Firstly, if any of these debts have not been paid in the last 6 years, they do not figure on your credit records and are unenforceable.

Secondly, if debts have been sold on, your original creditor will have no idea where they are now. Get your credit records and they will show you who you owe what.

Go via www.quidco.com and I believe you can get a free experian check, just remember to cancel within the 90 free period.

The only thing i can think of is a subject access rquest, but rewording it slightly.

You can alter this so that you are not receiving all the transaction detail for every account you have - going back 6 years but could be handy if you are reclaiming charges.

Please supply me with a copy of all information your company hold on me including a list of accounts, their status and balance and details of payments.

None of the debts are over 6 years old, Ive got a copy of my credit report which lists the names of the original creditors but I assumed that by now(3.5 years since making any payment) they would have been passed on to debt collectors.

Should I just right something like this then-

Dear bank

I took out a loan/creditcard with you in 2004 but have since defaulted on my repayments. I have since moved address and no longer have any information about this loan/credit card.
Please could you send information you hold about it to me so that I can arrange to repay it.
