Application for a job at Laura Ashley

hi,
I have an application form for Laura Ashley and I would loooovvve a job there but I just cant help feeling out of my depth. I went into the shop for a look around, especially around the furniture department as that where the job will be based and now Im trying to complete the application and I havent a clue.
First of all it says "Please give details of any serious or reoccuring illnesses you have suffered with" If I put that I have suffered with depression do yo think they will still consider me? I dont believe so, but I hate to lie... it doesnt mention a medical though.
The back of the form has questions on relating to attributes and stuff...

It must be the weather as Im on full steam ahead on the applications.
This is a letter I have written out for a job in a underwear shop.
Dear Sir/Madam.
I wish to be considered for the position of Sales Advisor as advertised in the Mall Vacancy section of the shopping arcade. As requested I have enclosed a copy of my c.v.
I am extremely approachable and friendly person with an excellent eye for detail. I have previous experience of working in a customer focused environment where I showed advanced problem solving skills in managing issues to the satisfaction of the customer within the boundaries set by the company.
I also have excellent communication and interpersonal skills with a wide variety of people and previous work experience has shown that I work well both as a member of a team and as an individual in achieving targets and getting tasks completed.
In addition I have a great deal of experience in dealing with both cash and credit/debit card payments gained in employment as a bar tender and shop worker, in these roles I have shown that I am both efficient and accurate. I also take great pride in my appearance and am reliable, hardworking and trustworthy.
I am seeking part-time employment as I am a mature student returning to study at the University of Central Lancashire in September. In the meantime I would be available to work increased hours and I am also available during all the holidays should I be required.

one addition (in bold), and I think a couple of your sentences should be shorter, they are too long, maybe punctuate here and there.
Lose the "many thanks for your time", and go with sincerely & appropriate name.
Otherwise personally I think it's excellent. Just remember you will be expected to back up everything you say with an example, go armed with that info and you should breeze it.Remember the time he ate my goldfish? And you lied and said I never had goldfish. Then why did I have the bowl Bart? Why did I have the bowl?0 -
OK OK - I may have **** luck with jobs but my letters usually get me an interview; to close, nearly always write:
"I would welcome the opportunity to discuss my suitability further; I can be available for (an) interview at your convenience. [This bit is optional] I look forward to hearing from you."
Your letters and you sound great; I'm sure you'll walk it!

Well, it's quite a posh shop. My sister has worked in retail for 10 years [worked her way up to senior merchandiser at BHS] and whenever she's applied she has never got passed the application stage.
If it's a company you would like to work for, make it a long term goal. So maybe start off getting some retail experience in a charity shop, then apply for retail work in department stores like BHS and Debenhams. Work your way into 'posher' companies like House of Fraser and John Lewis.
Never underestimate the benefits of networking as well. Go into your local store and get chatting to the sales advisers. Image/appearance is always a big issue in these kind of shops. Is there anything you can work on from that side as well. I've been guilty of this. I haven't worked for three months, and start my new job at a 5-star tourist attraction tomorrow. It's my first 5-star job so I've spent the last two hours scrubbing myself up. :eek:0
