Handing-in-notice Letter

Ive decided to bite the bullet and finally hand in my notice! But I have no idea how to word the letter since it's so long since ive done one.
I want to leave on good terms, I suppose so the doors always half open for me to return one day (if itwas poss).

lol thanks for the replies. im leaving b/c ive had enough of it really. the people that i work with have changed and ive also noticed a lot of the patients attitudes have changed also, they seem to think im a punch bag for all the stress they carry around. i have 2 children and would like to spend me time with them.0

Your emphasis should be on a desire to seek new challenges then. Something along the lines of "While I have grown professionally a great deal over the past X years with Bloggs Ltd, I feel it is now time for me to seek new challenges and a fresh working environment. I would like to take this opportunity to thank you for the experience and knowledge I have gained while working for you, and wish you all the best for the future."

If you know how much holiday you've got left for the amount of year you'll have worked, then I always think it's worth putting a bit along the lines of either
"I believe I am owed 5 days' holiday and I would like this to be added to my final salary payment"
OR
"I believe I am entitled to 5 days holiday and I would like to take this before I leave, therefore my last day at work will be (5 days before your month's notice ends)".Signature removed for peace of mind0 -

thanks for all the replies, i handed in my notice and am staying a couple of days. i dont particularly enjoy it there (i didnt say that though) but i know my job and its my children i want to concentrate on. i think the better time to look for a new ob is prob when they start high school, or i at least have a car and can get places lol0

It's tricky when leaving, esp if you want a reference later and you are concerned, but it can be really helpful for both sides if you can give some feedback about what didn't go well. Some places offer an exit interview, but of course not everyone is receptive to the truth. But as someone who has people working for me, I really want to know when things are not ok, like in any healthy relationship! THis kind of stuff should be coming out at reviews and appraisals though, it's always a bad sign if people are leaving unhappy and no-one knew or was able to prevent it.
