This offer is open to UK residents who visit https://www.ultralase.com and enter their name, address and contact information.

1. The Ultralase Prize Draw takes place on the last Friday of each month.
2. The winner is entitled to free laser eye surgery, worth up to £3,770 (£1,885 per eye).
3. Treatment is subject to a free, no-obligation consultation at which suitability for laser eye surgery is assessed and the most appropriate vision correction treatment type recommended.
4. Even though Ultralase has such a wide range of vision correction treatments, please note that around 10% of those attending consultations are deemed unsuitable for treatment.
5. This prize is not transferable under any circumstances, even if the prize winner is not deemed suitable for vision correction treatment.
6. The surgeon's decision as to whether or not to proceed is final.
7. There is no cash alternative.
8. Those who have already had vision correction treatment will not be entered into the draw.
9. Open only to UK residents.
10. Entrants must be over 18 years of age.
