Applying to have your interest paid without tax taken off

Thought I'd warn you all now, there is apparently a change in the requirements when applying to have your interest paid in whole.
My wife has just had her forms returned as she did not include her title. She had given her full name including middle name and all other details were correct, but because she had not used Mrs/Miss/Ms etc, the forms were not accepted.
Called bank to complain, as there is NO space on the form to input a title, nor is there any mention of it being required, it only asks for a name. Seems this is a new directive and the forms will be changed in the near future.
Save yourself some time and hassle now, include your title.

The new form is available here: R85 (2006) and Helpsheet - Getting your interest without tax taken off . The first field is "Saver’s full name and title"0

Kinda ironic that a forum member can supply a link to the correct updated form when a long-standing well known financial institution can't.
How much time would it take to update the form on their site for goodness sake?! They have basically made a simple process longer purely because of their inaptitude. (A&L, btw)0 -
Actually the form had been changed not in 2006, but in 2005  . See R85 and Helpsheet(LP) (2005) ...cpu wrote:...How much time would it take to update the form on their site for goodness sake?!0
