Re-Mortgaging with Bad Credit and credit card debt

Too cut a long story short.

On the help to buy scheme.

House Value £275,995
HTB Value: £54,999
Mortgage £195,500
Deposit just over £20k

Lived in the property for 2 years now and our fixed rate is coming to an end with Halifax, we haven't managed our credit well and I am looking at the best way of reducing my credit card debt.

Credit card debtween us is approx 15k
Loans £25k

The loans i feel are manageable well, but the Credit card debt is getting hard to manage effectively, I have missed a payment but generally keep it up-to-date.

Is it a possibility to take the equity out to pay off some of the debt?
