I don't think you can get one -

HMRC has stopped sending National Insurance number cards altogether. All customers (adults and young people approaching age 16) will now receive their National Insurance number on a notification letter.
You do not need to have a National Insurance number card - it is your number that is important.

They've stopped doing cards - I sent mine in for a change of name before the rules changed, a few weeks later they said they were no longer issuing cards but couldn't send my old one back

That website says

As announced by the Chancellor on 10 September 2010, HMRC no longer issues replacement National Insurance number cards. You don't need to have a card - it's the number that's important. If you can't remember your number, find out what to do by reading the section above.
