    It can be harder on a form than at an exit interview - when I left my last role I had both, and the answers to the questionnaire clearly formed the basis of the interview questions. I tried to be honest and objective, as I felt that it was important for my ex-colleagues and any new starters.

    While some things haven't changed, feedback from my ex-colleagues suggest that some of the issues are being addressed. Individual behavioural issues are being dealt with, and some organisation-wide issues have started to change too. I'm well aware that this isn't necessarily the result of my individual input, but talking to others who left at the same time, we all raised similar issues.

    During my exit interview five years ago I asked the HR person what happens to the information I give. The reply was 'nothing', it's an HR process / policy with no follow up.

    I left and did not take part in the process.
