Just looked at the web one and the entry form is there (Chrome) and goes to a "congratulations" page when done0 -

I spoke to next online support who said to delete cookies and use chrome. It still didn't work for me.....0

I just looked and the app and web forms still display for me - using
Incognito with Chrome or inprivate with Edge works with some forms that do not display although the main effect is the same as clearing cookies without the bother of doing it.0 -
