Bankruptcy Questions

Following on from a previous post:
I took out further credit whilst in an IVA. they have told me now that the IVA will fail. So I will now apply for bankruptcy.
1/ is there any chance that my application will get refused? If so what are the possible reasons?
2/ when I apply online and pay the fee what happens then? What I do do i need to send in etc etc.
3/ what kind of questions will the OR ask me.
Any other advice you think may be useful please share .
