Entrants to this competition must be aged 18 or over, a resident in the UK and hold an active UK Boots Advantage Card. Employees and the immediate families of Boots UK Limited, their agents, or anyone professionally connected with this competition are not entitled to enter the competition.An independent person, selected by Boots UK Limited, will randomly select the winner after 20th October 2013 from all valid entries their decision is final. This survey closes at midnight on 20th October 2013 - all entries must have been completed before this date.Entry into prize draw upon completion of Vitamins Survey. Only one entry per Advantage Card account. 5 winners will each receive 5,000 Boots Advantage Card points each which equals £50.00 to spend on products in store and on Boots.com, and they also will receive bags full of 1 month supply of selected vitamins. 30 winners will each receive 2,000 Boots Advantage Card points which equates to £20.00 to spend on products in store and on Boots.com, which will be added to your Advantage Card account. Boots Advantage Card points awarded as prizes cannot be exchanged for a cash alternative and are non transferable. Advantage Card points cannot be redeemed on Boots Opticians, Boots Hearingcare, prescription items, infant milks (up to 6 months), gift vouchers/cards, postage stamps, mobile phone - top up cards, Travel Services and Boots Insurance. The winners will be notified by 30th November 2013. Winners will be notified by email, telephone or letter. This must be acknowledged by the winner, by email, telephone call or letter and received by Boots UK Limited within 14 days of notification. Failure to respond to this email, telephone call or letter means an alternative winner will be selected from all valid entries received. It is the applicants' responsibility to ensure that their contact details are up to date on their Advantage Card account before the closing date. Advantage Card points will be available for collection at any Advantage Card Extra Offers Kiosk 7 days after the winner has acknowledged receipt of their winning entry. You will need to visit an Extra Offers Kiosk in a larger Boots store with your permanent Advantage Card within 6 months of this date to add these points to your card, after which time points will be lost.Responsibility will not be accepted for loss, delay and corruption of any data in transit or in the incomplete nature of any entries to the competition.
