Hi tcalamity,
Why not print a blank set off and start making notes by hand and then ask for advice if you get stuck. That way by the time you've got the information from your DMC you'll almost have it finished.
The Judge won't be interested in the minor detail of each account and providing you have all the names the O.R will be able to get hold of the statements and any other information they need.
Estimates of the amounts you owe will suffice if you really can't get hold of the exact amounts.
