My submission went through but only after I removed my phone number from the phone field and entered my e-mail address instead! It kept highlighting that field saying a valid e-mail address is required. Lousy form designing.

That might have invalidated my entry though...

Submitted after inputting email into phone field (same problem as Skilos).
Then to overcome the answer field I double spaced between the first and last name.
Guess that's what they mean by 'This field requires a unique entry and this value was already submitted.'
