Asking fellow DFW's for advice

Hi guys
I asked this in the reclaiming bank charges thread, but have had no reply, so I'm going to ask the lovely people I've spoken to on here instead.
When you are writing the first letter to reclaim bank/credit card charges, is the address you send it to your local branch, head office or the branch the account is held at? Also, can I claim from a joint bank account I held even though I'm no longer with the person I held it with?
