Worked OK for me (and I won a carton, thanks thatguy1 )

When clicking the link a Facebook pop-up appears asking you to click on 'Install' before it will let you enter your details. Once you do that it then enables the form. It's an Instant Win type of thing so they let you know if you've won or not as soon as you submit your details.
