Normally it's just name, address and telephone number. I'd go with subject as bafta goody bag but if it hasn't been specified no subject should be ok.

Hi ,no subject stated.In the paper it just asks for your details.My email is set up to add name,address & telephone number so that is what I sent.Up to each of though.
