Notice of Disassociation - Help?

Does anyone have any templates to do this? Do I need to send a fee with it to the 3 CRAs? I've spent the last 15 minutes searching but can't find anything specific. Basically I've fairly recently learned my soon-to-be ex wife has gone bankrupt, and I'd like to be disassociated sooner rather than later from that bombshell!

Hi RAS. Fortunately no. We had a joint loan a while ago but this was paid off when I took out a lower-interest loan with my own bank. Hi I have sent one in to Experian about two weeks ago and they said they were looking into it but nothing from them since .
Any idea how long they take to do this?
