Tax return following bankruptcy

Just wondering how to complete my tax return. I am self employed and was declared bankrupt last July. I've received a tax assessment to complete and I'm not sure how to go about completing it. Do I submit figures relating to my income following the bankruptcy? Would I attach a letter to the form explaining the situation?
Any ideas would be really appreciated.

    The first thing I would check is the UTR (Unique Taxpayer Reference) on the tax form.
    If HMRC know you were declared bankrupt the UTR should be different from the one you used in the past before you were declared bankrupt. If it isn't check with HMRC if you need to complete that one or a different one.
