On the last question it asks you for your email address so they can contact you if you win...

I didn't get that at all.
It asked how I listened (online) and then that was it.

Well the second time i didn't even ask me where I listened, survey just ended.

But it worked the third time.

You may need to lie even more. lol

The last part of the survey I had to put my email address as well. So my guess is you have to say yes for listening to Heart fm.
