1) The Ultralase Prize Draw takes place on the last Friday of each month
2) The winner is entitled to free laser eye treatment, worth up to £3,770 (£1,885 per eye).
3) Treatment is subject to a free, no-obligation consultation at which suitability for treatment is assessed and the most appropriate treatment type recommended
4) Note that 25-30% of those attending consultations are deemed unsuitable for treatment
5) This prize is not transferable under any circumstances, even if the prize winner is not deemed suitable for treatment
6) The surgeon's decision as to whether or not to proceed is final
7) There is no cash alternative
8) Those who have already had treatment will not be entered into the draw
9) Open only to UK residents.
10) Entrants must be over 18 years of age.
