I need to raise a grievance with my employer but can't find a decent template to use. Does anybody here know where I can download one from?

Look at the ACAS website https://www.acas.co.uk or telephone them on 08457 474747 .
In my recent experience they gave me the advice that was more appropiate to my situation and advised me how to take matters further if required.

your Grievance letter should detail all times, dates, and actions that you're complaining about, as such, there's no fixed format necessary under Employment Law, but if you DO proceed with a Grievance, do ensure that you follow the full three stage process, failure to do so will nullify any further action such as going to Employment Tribunal.
Trust me on this one, having helped my friend through this - it's vital to go through all stages. One further point,
if you go through the process, and your Employer doesn't follow the process from his side, as long as you can show this has happened, then you could still have a valid cliam at the ET. Oh, and whatever you do, make sure all communications are written down, take a friend or colleague or Union official with you to the meeting(s) -
Again, this will protect you if you have to go further.
