    I am trying to help ( long distance) complete this form SMIF003 for the mortgage interest. The person is on pension credit. I cannot find a 'blank' form online anywhere. Can someone advise where I can find one? I need it by Monday.Thanks

    Search online for a form MI12 this is the same.
