    Hello
    Thank you for entering
    I used to change address on-line but now I have forgotten the details of my Government Gateway User ID and password
    I do also have the D741 form, but I am not too sure if I fill it in, and send it to DVLA, along with my driving licence will be enough.
    Any suggestion according this matter will be very appreciative

    You can reset your Gov Gateway user id and password but if they need to use post it takes about a week.

    What worries me is that I need to send my old driving licence back and I can see the information on the website mentioned above are not updated, however I have found this number 0300 790 6801 for driver licence related enquirers, i will call the Monday
