CRB check for work .

Can anyone help please ?

I have to take my info. into work tomorow for my CRB check .

I have lost the form and i don't know what documentation I need to take .I can fill in the form when i get there .

Is there a link to a page listing what documentation is required?

CRB - can't find chapter and verse, yes I can, go to the site and do a search for CRB11 - applicant's guide blah de blah de blah

Just make sure that your documentation is in the right name, showing your current address ... very frustrating when people turn up soon after they've moved and say "but I haven't told DVLA I've moved yet" and expect me to accept their driving licence as proof of ID!

No, you need at least 3 pieces of ID documentation, 5 if you can't provide anything from the part of the list which includes passport and driving licence!

Mind you I should imagine the OP has now done it!

No ,I haven't .Had to take a sick day .

Thanks ,I gathered that Passport and Birth Certificate were needed.

I'm a bit peeved as ,I was checked last year but ,they lost it !!!
