Leeds BS ISA reactivation form

When I log into Leeds BS to transfer some money into my ISA, it says:

  • To pay funds into this ISA we require an ISA re-activation form.

If anyone has a link to a PDF of this form, please can you share it here?  It will save me ringing them and waiting for it to arrive in the post.

    I just hate ringing companies :D

    For anyone else searching for this form, I did ring them and asked them to e-mail it.  It arrived within an hour or two.  It's pre-filled with my information, so looks like they don't issue blank ones.

    The guy said he would also post me one.  When I asked how long the reactivation process takes, he said 7-10 days after they receive the form.  Today is Friday and I think the earliest they would receive the form is Wednesday.  So I put my £20k into one of my ISAs with a different institution so it can start earning tax-free interest from today and might ask Leeds to transfer the money in instead... their rates are good but their user experience isn't great.
